Family Caregiver Support Program
Services include:
- Information & assistance
- Options counseling
- Powerful Tools for Caregivers classes
- Trualta – Family Caregiver Education and Support Platform
- Respite care
- Supplemental services
- Monthly support group for those caring for a loved one 55+
- Monthly caregiver newsletter
Eligibility:
- Durham County resident (both caregiver and care recipient)
- Care recipient must be 60+ and caregiver must be 18+
- Diagnosed with Dementia or Alzheimer’s and/or limitations to Activities of Daily Living (ADL’s)
- Not receiving in-home support from other publicly funded programs
