The CNA performs duties as necessary for the safe and efficient operation of the Adult Day Health Program, works cooperatively with the Registered Nurse and program staff to maintain excellent standards of client care and thorough assigned records. The CNA 1- works closely with the RN, LPN, MSW, and the Director in carrying out assigned duties. This position reports directly to the RN and the Director of Adult Day Health.

 

Job Summary:

The PCA performs duties as necessary for the safe and efficient operation of the Adult Day Health Program, works cooperatively with the Registered Nurse and program staff to maintain excellent standards of client care and thorough assigned records. The PCA works closely with the RN, LPN, MSW, and the Director in carrying out assigned duties. This position reports directly to the RN and the Director of Adult Day Health.

RESPONSIBILITIES SHALL INCLUDE:

  • Maintains confidentiality of participant/staff health and related information.
  • Assists with toileting and personal care of clients
  • Leads recreational programs and Calendar of Activities programs as assigned
  • Assists with food service, including feeding, if necessary
  • Monitors participants for safety, status changes, debilitation needs and recommends/intervenes as appropriate
  • Files appropriate papers into participant folders
  • Performs other duties as requested or assigned by the RN and/or Director

Job Summary:

The Licensed Practical Nurse (LPN) performs nursing duties as necessary for the safe and efficient operation of the Adult Day Health Program, works cooperatively with the Registered Nurse and program staff to maintain excellent standards of client care and thorough assigned records. The LPN works closely with the RN, MSW, and the Director in carrying out assigned duties. This position reports directly to the RN and the Director of Adult Day Health.

This position is full-time up to 35 hours per week, Monday through Friday with flexible shifts between 8am and 5pm.

 

 

Responsibilities:

· Learning and applying the rules and regulations for the health care component of Adult Health Care services as outlined by the current Adult Day Health Care Standards of the NC Division of Aging and Adult Services.

· Complete preadmission health assessment for initial acceptance into the program, including issue identification and care planning

· Monitor participants responses to medical treatment plans and nursing interventions and revising plans of care as necessary

· Record and report results of nursing assessments, care rendered, and participant responses to care

· Collaborate with other health care professionals and caregivers regarding provision of participants health care

· Training and Educate Certified Nursing Aid staff on current industry best practices

 

Education and Experience Requirements:

· Completion of state approved training program (generally in the form of a diploma, certificate, or associate degree)

· LPN license

· BLS certification

· 1-2 years nursing experience
