Options Counseling Program

Options counseling provides guidance to individuals as they make informed choices about long-term services and supports. 

 

This service assists with understanding and considering various options while taking into consideration each person’s situation, values, resources and preferences. The options counselor can also assist with making an action plan for the individual to implement. 

 

Options Counseling is available to individuals who are 60 years of age or older or someone acting on behalf of a person age 60 or older. 

 

Options Counseling may be helpful for individuals who:

 

  • Do not know how to access long-term services and supports
  • Request transition assistance from one living setting to another
  • Lack awareness of existing community resources and supports
  • Want to plan for future long-term services and supports needs
